Holding a phone with a screen size of 30 cm/12 inches about 4″ from your eyes will give you almost accurate results. If you have a 7″ tablet, hold it 52cm/20 inches from your eyes.
Do not consider the tests in the application official tests. These tests are only meant to give you an idea of whether or not you should see an eye doctor or undergo eye treatment.
