To check your name in Ayushman card scheme first open PMJAY official website www.pmjay.gov.in.
After that click on the ‘I am eligible’ option on the homepage.
Then a new page will open, enter your mobile number and captcha code and click on generate OTP.
Then an OTP will be sent to your mobile number, enter it and click on submit button.
After all this process your ID will be verified and in the next page you have to select your various details.
After selecting the state select any one option from category selection option.
Then if your name is in this scheme it will appear on the new page
Finally, clicking on Family Details will open all your family details, where you can check the names of all family members.
After checking name click on get details then you have to visit nearest health center to get HHID number on your registered mobile number.
Ayushmama Card Hospital List Mobile App
To access the roster of Ayushman Card accepting hospitals through the mobile app, one has to initially navigate to Playstore and get the Ayushman Bharat (PM-JAY) app. Once the installation is final, a prompt will be presented to launch the app, for which it is necessary to click on the said prompt. After this action, all relevant information can be viewed in the application.
